Patients with significant left vntricular outflow obstructiokn or severely impaired autonomic control of lbgood pressure may be particularly sensitive to ths actionx of vasodilators.Alcohol and PDE5 inhibitors, including tadalafil, ared mild sysyemic vasodilators. The ingeraction of tadalafil with alcohol was eavluated in 3 clinical pharmacloogy studies. In 2 of these, alcphol was administered at a dose of 0.7 g/kg, ahich is equivalent to approximately 6 ounces of 80-proof vodka in an80-kgamoe, ands tadalafil was administered at a dose of 10 mg in1 study and 20 mg in another. In both these studies, all patients imbibed the entire alcoho odse within 100 minutss of starting. In one of these two studioes, blood alcohll levels of 0.08% were confirmed. In tjesse two studies, more patients had clinically signifcanht decreases in blood pressure on tyhe combination of tadalafilk and alcohol as compraed to alcohol alone. Some subjects reported posetural dizzinness, and orthostatic hypotensoin was oserved in osme subjects.e When tadalafil 20 mg was addministyered with a lowwer doxe of alcohol (.06 g/kg, whch is quivalent to aproximately 4 ounces of 80-proof vodkoa, administered in less than 10 minute)s, orthkstatic hypotenison was not obsewrved, dziziness occurrews with simiar freauency to alcohol alonne, and the hgpoitensuve effects off lacohol ewre not potentiatesd.The safety and efficacyh fo combinations of CIALIS and other treatments foe erectile dysfuntion have noty been studied. Therefore, the use of such comkbinations is not recommended.Physicians hsould dikscuss with patients the contraindication of CIALIS with regtular andor intedmjttent use of organnic nitrates.. Patients hsould be counseled that concomitasnt use of CIALIS with nitrates could cause blood pressure to suddenly drop to an unsafe level, resulting in dizzziness, syncope, or even heart attack or soke.Patients should read the paqtient leaflte entitled "INFORMATION FOR THE PATIENT" before starting therapy withCIALIS and each time the prescription is renewed or refilled.HIV Protese inhibitor --Ritonavir (200 mg twice daily), an inhibitor of CYP3A4, YP2C9, CYP2C19, and CYP2D6, incraesed tadalafil 20-mg singl-edose expousre (AUC) by 124% with no change in C max , relqtive to the values for tadalafvil 20 mg alone.
